When Hannah Sachs went for her 22-week ultrasound, her pregnancy had been progressing normally. Then doctors delivered unexpected news: her unborn son was suspected to have an encephalocele, a rare neural tube defect in which brain tissue or its protective membranes protrude through an opening in the skull.

Hannah, then 32 and living in Cincinnati, Ohio, with her partner Gabriel, was terrified that she might lose her baby. 💔

👶 A Newborn With an Unusual Beginning

Liam was born in March 2023, about two weeks before his due date, without major complications during delivery. But there was something immediately noticeable about him.

A large, fluid-filled sac—resembling a bubble and lovingly nicknamed “Squishy” by his family—protruded from his forehead between his eyes.

Liam was quickly transferred to the NICU, where he spent 13 days. Even after returning home, he needed supplemental oxygen because his oxygen levels frequently dropped.

💙 A Life-Threatening Complication

Only two weeks after coming home, Liam developed severe hydrocephalus, a dangerous buildup of cerebrospinal fluid inside the brain that can cause serious neurological damage.

At only about one month old, he underwent emergency surgery to have a ventriculoperitoneal (VP) shunt placed to drain the excess fluid.

Despite everything he was facing, Liam continued to show remarkable strength.

“He was already proving to be so strong at such a young age,” Hannah later recalled.

🏥 An 11-Hour Operation

Doctors knew Liam would eventually need a much more extensive procedure. Once he was old enough, surgeons planned to remove the encephalocele and reconstruct the severely affected areas of his skull and face.

In September 2023, Liam underwent an approximately 11-hour operation at Cincinnati Children’s Hospital.

A specialized craniofacial and neurosurgical team successfully removed the encephalocele and reconstructed much of his skull and facial structure. Because of the location of the defect, surgeons were also able to correct a significant portion of the ᴀssociated skull abnormality.

The procedure marked a major milestone in Liam’s journey. 🌟

🧸 Growing at His Own Pace

Today, Liam is a smiling and active toddler who continues to make progress in his own way.

He lives with cerebral palsy and will require his shunt for life, along with ongoing therapies and medical care. Yet his mother sees beyond the challenges.

She lovingly describes Liam as a “true miracle” and “imperfectly perfect in every way.”

“This life is very different from what I thought it would be, but it is more beautiful and rewarding than I ever thought possible,” Hannah has said.

For her, Liam’s journey is not about comparing him with other children. It is about celebrating every milestone he reaches on his own timeline. 💙

🧠 Understanding Encephalocele

Encephalocele is a rare birth defect that occurs when the neural tube does not close completely during early fetal development. In frontonasal or frontoethmoidal encephalocele, the type described in Liam’s case, the abnormality can create a visible sac or swelling around the forehead or face.

Treatment generally involves surgical repair, often during infancy, with the goals of protecting brain tissue, restoring the skull’s structure, and reducing potential complications.

Outcomes can vary considerably depending on the size and contents of the encephalocele and whether other conditions—such as hydrocephalus—are present.

Hannah has openly shared the emotional journey behind Liam’s story: the fear after the prenatal diagnosis, the trauma of seeing her newborn intubated and undergoing repeated procedures, and the complicated emotions that came with facing a future very different from the one she had imagined.
