For nearly a decade, Derek Pfaff lived with consequences most people could never imagine.

He struggled to eat normally. He could not blink. Speaking and breathing were difficult. And the face he once recognized in the mirror had been dramatically changed.

Derek was only 19 years old and a college student in Michigan when he survived a suicide attempt in 2014 that severely damaged most of his face.

When he eventually woke up in the hospital, he had no memory of what had happened.

He believed he had been in a car accident.

What followed was a nearly decade-long journey of reconstruction.

Derek underwent 58 reconstructive surgeries, with doctors working to restore as much function and appearance as possible. But despite years of treatment, major challenges remained.

He could not chew and swallow normally. He struggled with speaking and breathing. Most remarkably, he was unable to blink.

Eventually, doctors told Derek’s family that there was one major option remaining:

A face transplant.

The procedure would be extraordinarily complex, requiring an enormous medical team and hours of painstaking work.

In February 2024, surgeons at Mayo Clinic took on that challenge.

The operation lasted more than 50 hours and involved more than 80 medical professionals. Doctors replaced and reconstructed approximately 85% of Derek’s face, including his nose, jaws, teeth, eyelids and much of the facial tissue below his eyebrows.

Afterward, the results were about far more than appearance.

Derek could blink again.

He could smile.

He could swallow.

He could breathe through his nose.

For someone who had spent years struggling with basic functions most people take for granted, those changes represented an extraordinary new beginning. 🥹❤️

But one of the most emotional moments came when Derek finally saw his new face.

After everything he had endured, he looked at himself and said:

“After all that work, I look like a person again. It was incredible.”
