The shocking Times Square attack did not appear out of nowhere.

Before 49-year-old Pamela Cisneros was fatally sH๏τ by police after allegedly stabbing two strangers—killing 32-year-old Erin Piacenti and wounding 68-year-old Tak Kam—there was a troubling history that stretched back years.

According to her father, Cisneros experienced a severe mental-health crisis in 2019, when she attempted to jump in front of a subway train in Queens. Police records also describe a separate encounter in which she was found screaming and acting belligerently on Lexington Avenue.

Her family then tried to keep her stable. Her father said he began accompanying her to monthly hospital appointments where she received injections that appeared to help. For years, he believed she had turned a corner.

Then came April 2026.

Her father said Cisneros refused his offer to drive her to a regular mental-health appointment at Jamaica Hospital Medical Center, telling him she could go on her own and reᴀssuring him: “Don’t worry about me. I’m good.”

Months later, investigators say she walked through one of the busiest public spaces in America carrying two kitchen knives.

On August 31, police said Cisneros attacked Tak Kam and, seconds later, fatally stabbed Erin Piacenti. Officers confronted her minutes afterward. She refused repeated commands to drop the knives, and after attempts to use Tasers failed, officers sH๏τ her when she rushed toward them.

Now the most uncomfortable question is not simply what happened in Times Square.

It is what happened during the years in between?

A documented crisis. Treatment. A period her family believed represented recovery. Then a missed appointment—and ultimately a ᴅᴇᴀᴅly confrontation in the heart of Manhattan.

Authorities have not established that stopping treatment caused the attack, and the precise motive remains unclear. Officials have described the stabbing as random and unprovoked.

But the timeline raises a broader question that reaches far beyond one case:

When someone with a serious history begins slipping through the cracks, how can a family—or a system—know when a crisis is returning before it becomes irreversible?

And now, even Cisneros’s death is under investigation. New York Attorney General Leтιтia James’s office has opened a routine inquiry into the police shooting, as required by state law.
