Newly released toxicology and coroner records are providing additional details about the final hours of Addilyn “Addi” Smith, an 11-year-old Utah girl whose life ended during a February trip to Las Vegas with her mother.

Addi and her mother, 34-year-old Tawnia McGeehan, had traveled to Las Vegas for a cheerleading compeтιтion. Addi was scheduled to compete with Utah Xtreme Cheer.

She never made it to the event.

When Addi failed to appear and relatives were unable to reach either her or her mother, concern grew. On February 15, police conducted a welfare check at the Rio H๏τel & Casino.

Inside the room, investigators found Addi and McGeehan together on a bed.

Both were d*ad.

Authorities determined that Addi had been fatally sh*t and ruled her d*ath a homicide. McGeehan also died from a gunsH๏τ wound, and her d*ath was ruled a su*c!de.

Now, newly released records are revealing more about what investigators found.

According to Clark County Coroner’s Office records, multiple medications were detected in Addi’s system, including quetiapine, commonly known as Seroquel, gabapentin, lorazepam, known as Ativan, and diphenhydramine, or Benadryl. Caffeine was also detected.

The medications themselves were not listed as the cause of d*ath.

McGeehan’s toxicology results also showed several medications and other substances.

Perhaps most disturbing are the handwritten notes investigators say McGeehan left behind.

One note reportedly warned anyone entering the room to call 911 and indicated that two bodies, including an 11-year-old child, were inside.

Another note reportedly said Addi had been “very medicated” before her d*ath and had been sleeping beside her mother.

Investigators also said McGeehan intentionally left Addi’s phone accessible so that pH๏τographs and videos of the young girl could be viewed.

The records provide additional background on McGeehan as well. Family members reportedly told investigators that she had struggled with severe depression and had previously experienced su*c!dal thoughts.

The coroner’s report also referenced an ongoing custody dispute involving Addi’s father, Bradley Smith. McGeehan and Smith had reportedly been involved in years of custody litigation following their 2015 divorce.

For Addi’s loved ones, however, none of those details can change the most important fact.

She was 11.

She was supposed to be in Las Vegas cheering with her teammates.

She should have been thinking about routines, friends, school, birthdays and everything else waiting for her as she grew older.

Instead, her family received news no parent should ever have to hear.

Utah Xtreme Cheer later remembered Addi as a deeply loved member of its cheer family.

Behind every investigative document was a little girl who had a team, a family and people who loved her.
