Plot Summary

Years after the events of Last Blood, John Rambo lives in quiet anonymity in the mountains of the Pacific Northwest. Haunted but exhausted, he tries to put violence behind him.
But peace never lasts.

A classified CIA program called Project Sentinel—responsible for covert operations during the Vietnam War—has resurfaced. The leaders of the program are erasing all evidence of their illegal past, eliminating anyone tied to the project.

One of their targets: Sarah Tran, the daughter of a Vietnamese resistance fighter who once saved Rambo’s life. Now an investigative journalist, she uncovers documents linking her father, Rambo, and a secret black-ops unit called Ghost Squadron.
Before she can publish the truth, her team is massacred by Sentinel operatives.

Desperate, she tracks down the only person who can protect her: John Rambo.


Rambo Returns to War

At first, Rambo refuses to fight. He believes the world has no place left for men like him.
But when Sentinel forces attempt to kill Sarah—and destroy Rambo’s home in the process—the warrior inside him awakens.

Rambo uncovers the horrifying truth: Ghost Squadron was used as expendable soldiers for illegal missions, and Sentinel’s new commander, Colonel Vess, plans to restart the program using enhanced soldiers trained through psychological torture.

Rambo realizes he isn’t fighting for revenge—he’s fighting to stop a new generation from suffering the same fate he did.


Climax

Rambo tracks Colonel Vess to a fortified mountain base built from repurposed Cold War bunkers.
Using his classic guerrilla tactics—traps, tunnels, improvised explosives—Rambo wages a one-man war through forests, caves, and steel corridors, picking off Vess’s enhanced soldiers one by one.

In the final confrontation, Rambo faces Vess in hand-to-hand combat. Wounded but relentless, Rambo defeats him and destroys the base, ensuring Sentinel dies for good.


Ending

Mortally injured, Rambo collapses beneath the open sky. Sarah finds him, and Rambo tells her:

“Tell the world the truth. Don’t let them hide what they did.”

The film ends with Rambo walking into the forest sunrise—bloody but unbowed—implying his survival, yet honoring the mythic end of a warrior who never stopped fighting for others.

A final shot:
A young Marine recruits’ class listens to a story about a nameless soldier who fought every war alone—hinting that Rambo has become legend.
